Important to not discount the pressure placed on surgeons by referrings to operate. Unfortunately those applying the pressure often the same ones to judge when outcomes predictably not so good. And there is no specialty under greater public scrutiny!
Lovely clear example of the need for: 1) Good evidence incorporating outcomes relevant to the patient (PROMs) 2) Clinicians with the skill to explain the evidence to support decisions 3) Recognition that heuristics and hubris can hamper decision making
